Samuel was born in Glossop, Derbyshire, on the 31st December, 1893. He enlisted at Valcartier Camp, Quebec, 20th September, 1914, stating his occupation as farmer, and his next of kin as Joseph R. Booth, of 41, Thorpe Street, Glossop, Derbyshire.
He was five feet seven inches in height, had a fair complexion, blue eyes and light brown hair, and he belonged to the Church of England. He had two vaccination marks on his right arm.
